Personnel Training and Records

Purpose
Records of credentials and training pertinent to each designated position within the biorepository are necessary for employment.
Scope
All employees of the Neuropathology Biorepository must provide credentials and records upon hiring. Included in these records are records of training to ensure competency in all instruments/methods applicable to their designated job. Competency is assessed and corrective action is in outlined.
Personnel File
A personnel file is kept by the biorepository manager for each employee of the biorepository. Included in this file are:
- Summary of training and experience
- Copy of academic degree or transcript, if applicable
- License, if required by state
- Certification, if required by state or employer
- CITI Certification
- Description of current duties and responsibilities as specified by the biorepository director:
- Procedures the individual is authorized to perform
- whether supervision is required for specimen processing, testing performance or result reporting
- whether supervisory or director review is required to report participant results
- Records of continuing education
- Records of radiation exposure where applicable
- Not required for low exposure levels
- Work-related incident and/or accident records
- Dates of employment
Training and Assessment
All laboratory personnel receive on the job training. Individual competency is measured by supervisory observations. Yearly performance appraisals are used to document competency.
- All new employees are oriented by the supervisor on the first day of employment. An orientation list is reviewed and signed by the new employee and supervisor and forwarded to the biorepository manager. The new employee is given a copy of the current Laboratory Safety Manual.
- All new employees are in a 2 to 3 month probationary period under review by the Neuropathology Biorepository staff. New technologists must read all general SOPs and the technical SOPs that have been assigned prior to being trained at the bench. This will ensure that the new employee is familiar with the clinical applications of the test, the test principles and the test procedure.
- A supervisor trains the new employee on each test. The new employee first observes the test and then performs tests under the supervision of the trainer. Finally, the new employee performs the test independently, but with the supervisor available to address any questions.
- A training checklist for each test is given to the new employee to review. Tasks are listed within each service. When the new employee is trained on these tasks, the checklist item is signed and initialed by the training supervisor. The checklist is signed and kept on file.
- If competence has not been demonstrated, it will be determined by the director and/or management of the biorepository whether to extend the probationary period for further training, or to disapprove a permanent position. This will be documented on the training checklist and kept on file.
